Replacement for Damaged Passport

Required documents

Especially for those whose passports are considered corrupted by the Embassy of the Islamic Republic of Iran for reasons such as tearing, drainage, etc.

  1. Completion of Form for Passport Issuance (Form G1 )
  2. Completion of the Citizenship Survival Form ( G2 Form) for those over 18 years of age and non-Iranian passports
  3. Completion of application form for Exit Seal ( G6 ) for men serving
  4. Completion of the confused passport application form (Form G5 )
  5. The principle of previous passport and clear and legible image of the profile pages and the last stamp of departure from Iran
  6. The principle of the duty system card (for men) and a clear and legible image on the back and on it
  7. Principle and Image of Valid Residence Certificate (Visa or Non-Iranian Passport)
  8. New colour photo (passport size) Full face with white background without glasses, hat and scarf, face roundness (3 pieces)
  9. Original and Image ID of a series of all pages on paper (ID of persons over 15 must be photographed) A4
  10. Customized Envelope Envelope to Return Passport
  11. Consular service fee:
    £140 for people under 18 (£160 if you apply for a change of residence in England)
    £140 for people over the age of 18 (£160 if you apply for a change of residence in England)
    £20 Inquiry Date of last departure from country
    £35 for people who intend to use the seal for the first time per year.(Those who have used it in recent years pay only £10 for a new seal)

All forms must be completed and signed in full, and if the forms are incomplete, unsigned, or incomplete, the documents will be returned without action. All forms must be signed by the father or paternal grandfather or legal guardian. A parent or guardian is required for children under the age of 18 or spouse to issue a first passport or change of residence at the consulate. Special exclusion applies to men 18 years of age or older who have no end of service or exemption card, have been abroad for at least three years. All copies requested should be made clearly and legibly in A4 paper. The inclusion of the country of residence in the United Kingdom in the new passports constitutes frequent departure and no longer requires a seal of departure. The presence of people at the embassy to obtain an Iranian passport for the first time as well as a missing passport is required. Students who first apply for this passport should apply for a passport renewal in person with the documents in hand. Providing original proof of identity such as birth certificate, end-of-service card or exemption is mandatory, and if you do not have proof of origin, ask a relative to submit your identity documents to the Ministry of Foreign Affairs in Tehran (Imam Khomeini Square Building 3, Passport and Visa Office). Or the Foreign Ministry’s representative offices in most major cities, such as Shiraz, Isfahan, Tabriz, Bushehr, and Mashhad, etc. to send the image to the agency through the said office. Men over 50 years of age do not need to send a copy or end of service card exemption or exemption

  1. Make sure you meet the criteria and have all of the documents and forms (completed) listed in the requirements above.
  2. FOR ONLINE APPLICATIONS you will need to have scanned (or photographed) copies of your documents at hand. You can upload these on the Checkout Page of this website as you proceed with payment of the fees.
  3. Select between the APPLY ONLINE or APPLY IN-PERSON red button options below.
  4. ONLINE APPLICATIONS (£20 online processing fee applies):
    • After clicking the red "Apply Online" button, select your application type and add it to the cart to pay the fees.
    • In the checkout, using the tool provided, upload your scanned documents / photo's.
    • Upon completion of the payment - you will recieve an Order Reference Number via email.
    • Our team will be in-touch to offer you advice and/or further instructions (if needed).

*Online applications require the uploading of douments and photographs as stated in the application requirements. In some cases online application is not possible as original documents are required.
